Doctors have offered theories, fans have scrutinized her appearance, and Madonna has challenged her critics. Now, AI imagines a different path for her face.
Long before Madonna became one of pop music’s most recognizable figures, she struggled with how she saw herself.

In a mid-1980s interview with actor Harry Dean Stanton, later republished by Interview magazine, Madonna spoke candidly about the insecurity she carried as a teenager. Before finding a sense of purpose, she said, she questioned both her appearance and her abilities.
"Before I started feeling devoted to dancing, I didn’t really like myself very much," Madonna recalled. "I didn’t think I was beautiful or talented."
She said she spent much of that period feeling that she did not fit in at school and resenting her parents’ authority. "I really had a lot of self-hatred," she said.

Dance gave her a way to redirect those feelings. Madonna described herself as "unmolded clay" and said she gradually realized that work, discipline, and help from others could allow her to transform herself.
"When I started having a dream, and working toward that goal, having a sense of discipline, I started to really like myself for the first time," she said.
That growing confidence became part of a broader outlook. Madonna told Stanton that she believed in herself because she had started as "a little girl from Michigan" with a dream, worked hard, and watched that dream become reality.

Born Madonna Louise Ciccone on August 16, 1958, in Bay City, Michigan, she moved to New York in 1978 and pursued dance, according to IMDb. She studied with choreographer Alvin Ailey and later joined the Patrick Hernandez Revue.
Her ambitions soon expanded into music. Madonna became involved with Breakfast Club and later worked with Stephen Bray on dance-oriented recordings. Producer and DJ Mark Kamins eventually passed her demos to Sire Records in early 1982.

Experts Examined Madonna’s Face – and Saw Several Possible Changes
By October 2022, speculation about Madonna’s changing appearance had intensified. Page Six consulted three medical and aesthetic professionals, who compared photographs of the singer and offered their views on the cosmetic work she may have undergone.

Dr. Ehsan Ali, Dr. Ramtin Kassir, and physician associate Pamela Weinberger had not treated Madonna. Their assessments were based on photographs of the singer rather than firsthand knowledge of the procedures she may have undergone.
Ali believed several features pointed to possible cosmetic work.
"She for sure had a facelift," Ali said. "You can see her skin is pulled back and tightened, with no loose skin and no wrinkles. A facelift also alters the shape of your eyes, which you can see in her image, her eyes seem ‘pulled back.’"

He also suspected that her nose had been surgically altered and believed Botox could explain the appearance of her forehead and the areas around her eyes and between her eyebrows.
Kassir independently focused on Madonna’s nose. He said it appeared unusually narrow, with a more pointed tip than in earlier photographs. But the nose was only one part of the change he believed he could see.
Her facial proportions also caught his attention. Kassir described Madonna’s overall facial shape as more exaggerated, with cheeks that appeared particularly high and full.

The fullness of her cheeks led Weinberger to a different part of the cosmetic discussion: filler.
She believed repeated filler treatments may have created excess volume in Madonna’s cheeks and temples. Too much filler, she explained, can begin to obscure the underlying structure of a person’s face rather than simply restore lost volume.
Ali also believed Madonna’s cheekbones looked "completely different" from when she was younger. He suspected filler had contributed to their more prominent appearance. The experts then moved beyond the center of her face.

Weinberger and Kassir both raised the possibility of a brow lift after examining Madonna’s brow and hairline. Weinberger also suggested Botox in the masseter muscles may have helped create a narrower appearance through the jaw.
Ali focused instead on the definition of Madonna’s lower face. He believed the sharpness of her jawline could be consistent with liposuction beneath the chin and a neck lift.
Kassir nevertheless believed the overall effect was significant. "Overall, she looks like she has taken plastic surgery and aesthetic procedures a little too far," he said.

Then One Grammys Appearance Changed the Conversation
Madonna’s appearance at the 2023 Grammy Awards brought another surge of commentary about her face.
On X, one netizen wrote, "#GRAMMYs I really wish Madonna never touched her face. There really was no reason. She was going to age beautifully. She’s always been beautiful. #Stopplayingwithyallface #Grammys2023 #Madonna [sic]"

Another commenter connected the issue to Madonna’s history of challenging convention: "She could have redefined what growing older looks like as she redefined a lot of other things. Guess I was kinda hoping for some trailblazing here…"
Others questioned whether the public scrutiny itself was part of the problem. "I wonder if it has anything to do with the constant commentary on her appearance from people who should know better," one user wrote.
Another remarked, "Such a shame!" A fifth commenter wrote, "It was pretty cruel of them to give comparison photos behind her. She really was aging well. It’s so sad she didn’t see it."

The reaction continued on Instagram. "Noooo more stuff in the face please my sexy star since I was a Lil girl…..girls just wanna have fun. Please your already beautiful no more needle…no disrespect [sic]," one person wrote. Another asked, "So sad 😢 what happen to you?"
One commenter claimed, "This is so sad to see her looking this way, she’s had that much work done her eyes don’t move and she looks like she’s got zero movement in her face, come on Madge you once said you would never have plastic surgery what the hell happened, you don’t look good hun you look like a wax work."
Another wrote, "Just horrific…why did she do this to herself. She looked better before." A different user said, "She looks scary and I am going to have nightmares tonight…she was so beautiful, why would she do this to herself?!?!"

"You have ruined yourself. You dont even look like yourself[sic]," another wrote, while someone else commented, "Wow eyes give it away."
One longer response suggested public pressure had played a role: "Daw, you really fell victim to the pressures of self and the public eye to where you’ve injected your face to the cheap standards of modern day sold expectations? 😢 I’d defend your right to freedom if only you remained free…."
Madonna Had a Very Different Explanation for Her Appearance

In a February 2023 Instagram post, Madonna said the fixation on her appearance had overshadowed the reason she was at the Grammys.
She had introduced Sam Smith and Kim Petras before their performance. Madonna described Petras’ appearance as a "History making moment!!" and said she wanted attention focused on artists willing to take risks.
Instead, she said, many people discussed close-up photographs of her taken with a long-lens camera that "Would distort anyone’s face!!"

"Once again I am caught in the glare of ageism and misogyny That permeates the world we live in," Madonna wrote.
She argued that society refuses to celebrate women past 45 and instead punishes them when they remain "strong willed, hard-working and adventurous."
Madonna also made clear that she did not intend to change her approach because of the criticism.

"I have never apologized for any of the creative choices I have made nor the way that I look or dress and I’m not going to start," she wrote.
Days later, she addressed the attention with humor. In an X post, Madonna shared a photo of herself seated with her arms crossed. She wore a black cap, dark jacket, ripped jeans, and long orange braids.
"Look how cute i am now that swelling from surgery has gone down. Lol 😂," she wrote.

Two Years Later, a Surgeon Noticed Something Had Changed
In May 2025, plastic surgeon Dr. Steven R. Cohen reviewed more recent images of Madonna following the Met Gala and compared them with her appearance roughly two years earlier.
"Two years ago, she looked overfilled," Cohen said in a TikTok video. "Now, she looks normal."

The difference led Cohen to consider what might have changed. He questioned whether some of Madonna’s filler had been reversed, but he also raised another possibility: weight loss associated with a medication such as Ozempic.
Cohen did not claim that Madonna had dissolved filler or used the drug. Instead, he presented both as possible explanations for the changes he believed he could see in photographs.
His attention then shifted to specific areas of her face. Cohen noted some hollowing around the upper and lower eyelids, along with extra skin and deeper nasolabial folds – the creases that extend from the sides of the nose toward the corners of the mouth.

Then, in August 2025, Page Six reported that Madonna may have been moving toward a less sculpted appearance.
An unnamed insider claimed her boyfriend, Akeem Morris, had encouraged her to move away from fillers and augmentations.
"She’s starting to listen to Akeem, who tells her she’s beautiful," the source said. "She doesn’t normally listen to anyone but she does him." According to the insider, Madonna wanted to "own being 67 rather than trying to look 27."

The source said she was instead trying LED lights, oxygen facials, and lymphatic drainage. "Her face is so iconic that it needs to look like her," the insider said. The source added that Madonna wanted her appearance to look "more natural," "less sculpted," and wanted her face "to move."
Still, it raised a question that years of photographs and public debate cannot answer: What might Madonna look like now if her face had aged differently?
